The dual epidemic of TB and HIV, combined with the rise in multidrug-resistant TB (MDR-TB), placed immense pressure on Namibia\u2019s public health system. Many patients\u2014especially those in remote regions and underserved communities\u2014faced delays in diagnosis, limited access to treatment, and challenges in completing therapy.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">In response to this urgent public health need, <strong>Neftaly implemented a two-year TB control initiative in partnership with Namibia\u2019s Ministry of Health and Social Services (MoHSS)<\/strong>. The goal was to expand access to high-quality TB and MDR-TB services and to strengthen the capacity of the national TB program to deliver sustainable, patient-centered care.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Program Objectives<\/h3>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Improve early detection and accurate diagnosis of TB and MDR-TB<\/li>\n\n\n\n<li>Expand equitable access to high-quality treatment and care<\/li>\n\n\n\n<li>Support effective TB\/HIV collaborative activities<\/li>\n\n\n\n<li>Build the capacity of healthcare workers and health facilities<\/li>\n\n\n\n<li>Strengthen the surveillance and monitoring system for TB control<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\">Key Program Components<\/h3>\n\n\n\n<h4 class=\"wp-block-heading\">1. <strong>TB and MDR-TB Case Detection<\/strong><\/h4>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Introduced <strong>GeneXpert technology<\/strong> for rapid diagnosis of TB and rifampicin resistance in key districts<\/li>\n\n\n\n<li>Trained laboratory staff and healthcare workers in <strong>sample collection, testing protocols, and quality assurance<\/strong><\/li>\n\n\n\n<li>Deployed mobile outreach teams to increase TB screening in high-burden and hard-to-reach areas<\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading\">2. <strong>Access to Treatment and Adherence Support<\/strong><\/h4>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Strengthened <strong>directly observed treatment (DOT)<\/strong> services in health facilities and communities<\/li>\n\n\n\n<li>Supported the rollout of <strong>second-line treatment for MDR-TB<\/strong> patients, including clinical monitoring and psychosocial support<\/li>\n\n\n\n<li>Improved treatment adherence through <strong>community-based care models<\/strong> and patient follow-up systems<\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading\">3. <strong>Integrated TB\/HIV Services<\/strong><\/h4>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Promoted <strong>routine HIV testing and counseling<\/strong> for all TB patients<\/li>\n\n\n\n<li>Provided <strong>co-trimoxazole prophylaxis and ART<\/strong> for eligible TB\/HIV co-infected patients<\/li>\n\n\n\n<li>Facilitated joint TB\/HIV training for service providers to ensure continuity of care<\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading\">4. <strong>Health Workforce and Facility Strengthening<\/strong><\/h4>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Trained over <strong>800 healthcare providers<\/strong> in TB case management, drug-resistant TB protocols, and infection control<\/li>\n\n\n\n<li>Supported health facilities with <strong>guidelines, treatment algorithms, and monitoring tools<\/strong><\/li>\n\n\n\n<li>Improved infrastructure in selected DOT centers and MDR-TB treatment units<\/li>\n<\/ul>\n\n\n\n<h4 class=\"wp-block-heading\">5. <strong>Monitoring, Evaluation, and Data Management<\/strong><\/h4>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Strengthened national and district-level <strong>TB surveillance and reporting systems<\/strong><\/li>\n\n\n\n<li>Introduced <strong>digital case-tracking tools<\/strong> to monitor treatment outcomes and patient retention<\/li>\n\n\n\n<li>Supported regular data reviews and operational research to inform program improvements<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\">Key Achievements (2011\u20132013)<\/h3>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>GeneXpert machines deployed in 7 districts<\/strong>, resulting in faster diagnosis and earlier treatment initiation<\/li>\n\n\n\n<li><strong>Case detection rates improved<\/strong> by over 20% in target regions<\/li>\n\n\n\n<li><strong>Treatment success rates exceeded 85%<\/strong> in supported sites<\/li>\n\n\n\n<li>More than <strong>1,500 patients with MDR-TB enrolled in care<\/strong> with structured follow-up and support<\/li>\n\n\n\n<li><strong>TB\/HIV integration expanded<\/strong>, with over 90% of TB patients tested for HIV<\/li>\n\n\n\n<li>Enhanced national capacity for TB program planning, supervision, and evaluation<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\">Conclusion<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Neftaly\u2019s TB program in Namibia demonstrated that targeted investments in diagnostics, healthcare workforce development, and community engagement can significantly improve TB outcomes.
