    Tuberculosis (TB) remains a critical public health challenge in Romania, which bears one of the highest TB burdens in the European Union. Despite progress in recent years, the emergence of drug-resistant TB (DR-TB) and systemic inefficiencies continue to threaten national TB control efforts. Neftaly is committed to supporting Romania in its fight against TB by contributing to the reform of its TB control system and enhancing the management of drug-resistant forms of the disease.

    Addressing the Core Challenges

    The current TB control system in Romania faces several key issues:

    • Fragmented service delivery between primary care, public health institutions, and hospitals
    • Insufficient diagnostic capacity, particularly for rapid drug resistance testing
    • Gaps in patient-centered care, leading to poor treatment adherence
    • Limited integration of social support and mental health services for TB patients

    These challenges are compounded in the management of multidrug-resistant (MDR) and extensively drug-resistant (XDR) TB, which require longer, more complex, and more expensive treatment regimens.

    Neftaly’s Strategic Response

    Neftaly’s strategy focuses on systemic reform and targeted capacity-building through the following pillars:

    1. Modernizing TB Surveillance and Diagnostics

    We support the scale-up of rapid molecular diagnostics (such as GeneXpert and line probe assays) to ensure timely and accurate detection of drug resistance. Enhanced electronic reporting systems will also enable real-time tracking and response.

    2. Improving Access to Quality Treatment

    Neftaly works with national partners to ensure that patients with TB and DR-TB receive WHO-recommended, shorter, and more effective treatment regimens. This includes improving drug supply chains, reducing treatment delays, and decentralizing care delivery.

    3. Strengthening Human Resources and Clinical Management

    Training healthcare providers on the latest TB and DR-TB management guidelines is key. Neftaly facilitates workshops and mentoring for clinicians, nurses, and community health workers across Romania, improving quality of care and case management.

    4. Promoting Patient-Centered, Community-Based Care

    Neftaly advocates for a shift from hospital-based to ambulatory, community-driven TB care. By involving civil society and patient organizations, we promote adherence, reduce stigma, and improve treatment outcomes.

    5. Policy Advocacy and System Reform

    Neftaly Tuberculosis (TB) Case Notification Improvement Project

    Objective:
    To increase the case notification rate of all forms of tuberculosis (TB) in the target population from 55.1 per 100,000 population in 2017 to 287 per 100,000 population by 2020.

    Project Overview:
    The Neftaly TB project was designed to significantly enhance TB detection and reporting across program areas, addressing underdiagnosis and underreporting that hinder effective TB control. Through strategic interventions, the project aimed to identify and notify more TB cases promptly, thereby improving treatment outcomes and reducing transmission.

    Key Strategies:

    • Strengthening active case finding (ACF) through community outreach and targeted screening, especially in high-risk and underserved populations.
    • Enhancing diagnostic capacity by equipping health facilities with improved laboratory tools and training healthcare workers in TB identification and management.
    • Raising community awareness about TB symptoms and encouraging early health-seeking behavior.
    • Building robust reporting and surveillance systems to ensure timely and accurate case notification to national TB programs.
    • Collaborating closely with local health authorities, community organizations, and partners to streamline case referral and follow-up.

    Expected Outcomes:

    • A substantial increase in notified TB cases, reflecting improved detection and reporting.

    Neftaly Achieves Milestone: 50,520 Tuberculosis Cases Detected

    Between July 2006 and June 2011, Neftaly successfully detected a total of 50,520 cases of tuberculosis (all forms) through its TB intervention programs in Niger. This critical achievement marked a major contribution to the national and global fight against TB, particularly in one of the world’s most vulnerable and underserved regions.

    Key Highlights

    • Comprehensive Case Detection
      Neftaly’s integrated approach—combining community outreach, improved diagnostics, and frontline health worker training—enabled the identification of 50,520 individuals affected by TB, including both pulmonary and extrapulmonary forms.
    • Early Detection Saves Lives
      Early diagnosis helped prevent the spread of TB and significantly improved treatment outcomes. Many of the cases were found in rural, high-risk populations with limited access to health services.
    • Community-Based Strategy
      The large number of detections was made possible through strong community engagement, mobile screening units, and door-to-door case finding campaigns, especially in hard-to-reach areas.
    • Targeted Screening Efforts
      Special attention was given to vulnerable groups such as children, women, nomadic communities, and people living with HIV, where TB incidence is often underreported.
