Neftaly Create: Enabling Conditions for Strengthened HIV Care Access and Continuity in the Region
Overview
Neftaly Create is committed to transforming the landscape of HIV care across the region by fostering enabling conditions at national and regional levels. Our goal is to ensure equitable, uninterrupted access to HIV services for key populations (KPs) — including sex workers, men who have sex with men, transgender individuals, people who inject drugs, and incarcerated individuals — in five participating countries.
Objective
To strengthen the continuum of HIV care by improving systemic access and reinforcing linkages between prevention, diagnosis, treatment, and long-term care for key populations through a coordinated, rights-based, and evidence-informed approach.
1. Enabling Conditions at the National Level
Policy and Legal Reform
- Advocate for decriminalization and reduction of legal and policy barriers that impede access to HIV services for KPs.
- Support harmonization of national HIV strategies with regional human rights and public health commitments.
- Engage stakeholders in national policy dialogues to ensure inclusive decision-making processes.
Health System Strengthening
- Integrate HIV services within broader universal health coverage (UHC) frameworks.
- Expand training for healthcare workers on non-discriminatory, gender-sensitive, and KP-centered service delivery.
- Improve data collection systems to disaggregate data by key population groups to inform evidence-based planning.
Community Empowerment and Participation
- Support the development and sustainability of KP-led organizations to participate in planning, monitoring, and service delivery.
- Promote social protection programs to address structural vulnerabilities such as stigma, poverty, and gender-based violence.
2. Enabling Conditions at the Regional Level
Regional Coordination and Knowledge Sharing
- Foster regional platforms for policy alignment, knowledge exchange, and cross-border collaboration on HIV care for KPs.
- Encourage best practice sharing through regional learning networks and South-South cooperation.
Technical Assistance and Capacity Building
- Provide regional training and technical support to national programs to scale up innovative and proven models for KP-friendly services.
- Facilitate peer-to-peer learning opportunities among implementing countries to strengthen HIV program quality.
Regional Monitoring and Accountability Mechanisms
- Develop and align regional indicators to track progress on KP-focused HIV interventions.
- Support regional civil society and KP networks to engage in monitoring and advocacy across national borders.
3. Strengthening Linkages Across the HIV Continuum of Care
Integrated Service Delivery Models
- Promote one-stop-shop models offering prevention, testing, treatment, and support services in community and facility settings.
- Strengthen referral systems and case management approaches to ensure continuity of care from diagnosis through to viral suppression.
Digital Health and Innovation
- Utilize digital platforms to support appointment reminders, virtual consultations, and self-testing initiatives.
- Expand access to telehealth and mobile clinics to reach under-served areas and populations.
Sustainable Financing and Donor Coordination
- Encourage national investment in HIV programs targeting KPs through domestic resource mobilization.
- Coordinate donor efforts to ensure aligned and complementary support for KP-focused services across countries.
Conclusion
Through Neftaly Create, we are laying the groundwork for a robust, integrated, and equitable HIV response. By enabling supportive policy environments and strengthening cross-country collaboration, we are building sustainable systems that place key populations at the center of care, improving health outcomes, reducing HIV transmission, and upholding the dignity and rights of all individuals affected by HIV.
